Keech Hospice are seeking a part-time Transition Senior Nurse (22.5 hours per week) to join our growing close knit team.  

.  

As a Transition Senior Nurse you will work with our established Transition Lead to support young people receiving hospice care and their families as they prepare for adulthood. You’ll be working across Bedfordshire, Hertfordshire and Milton Keynes offering compassion, expertise, and support every step of the way. 

 

  • Support the transition of young people to Adult Social Care, Education and/or heath services 

  • Offer advice to colleagues regarding transition issues 

  • Ensure appropriate advice, advocacy and information is provided to young people aged between 14 and 25 

  • Ensure Keech Hospice meets the required standard regarding transition of care  

   

Essential 

  • NMC Registered Adult, Children’s or Learning Disabilities Nurse 

  • Experience in working with young people and palliative care 

  • Experience of face to face working with young adults and families and managing a caseload 

  • An adaptable approach to a changing work environment 

  

Desirable 

  • Mental capacity & best interest assessment  

  • Teaching skills 

  • Advanced level communication skills
